 ABSTRACT

The surgical treatment is commonly used to treat malignant thyroid nodules but the surgical strategy depends on the evaluation of the malignance or benign of lesions. However, there has been controversial debate on the methods of definitive diagnosis before surgery. In the past, the ultrasound and Magnetic Resonance Imaging (MRI) methods had the significant value only for bilan evaluation. Given the robust high field MRIs from 1.5 Tesla, particularly with their diffusion pulse sequences which enable to calculate the apparent diffusion coefficient (ADC) have been expected to be a valuable method to differentiate benign from malignant thyroid nodules. The principle of this method is that malignant lesions have low ADC values while benign lesions have high ADC values in general. Nevertheless, relatively little research on the correlation between ADC values and the malignant thyroid nodules has been carried out so far. Specifically, there has been no research on this topic of thyroid in Vietnam.

Objectives

Description of diffusion - weighted images in thyroid nodules and initially assessment of the result.

Methods

Prospective study on 21 patients (29 thyroid nodules: 21 benign nodules, 7 malignant nodules) who were acquired the diffusion - weighted magnetic resonance imaging (DW-MRI) with 200, 300, 400, 500, 600, 700, 800 b values and ADC maps were calculated, fine needle aspiration cytology (FNAC) were performed in 21 nodules. Diagnosis confirmed by postsurgical histopathologic examinations.

Results

ADC values of nodules provide useful data about the nature of a thyroid nodule, with b200 DW-MRI, the mean ADC values of thyroid nodules were 1.45 ± 0.30 x 10-3 mm2/s in the malignant group and 2.26 ± 0.33 x 10-3 mm2/s in the benign group, with signicantly different (p <0.001). According to an ADC cut-off value of 1.98 x 10-3 mm2/s, the sensitivity, specificity, positive predictive value, negative predictive value, and accuracy in differentiating benign from malignant thyroid nodules are calculated as 100%; 81%; 66.7%; 100% and 86.2%, respectively. The DW-MRI has the higher values than FNAC for diagnosis of thyroid cancer.

Conclusion

ADC values on the diffusion - weighted imagings (DWI) are significant to differentiate benign from malignant thyroid nodules. We can applied DW-MRI for diagnosis of thyroid nodule in the hospital which do not have frozen section biopsies’s practice.
